Case Management

Case management is open to those in need, regardless of whether or not they live in south Kansas City.  Priority for case management is given to homeless individuals and families, and those at high risk of homelessness.
 
At the beginning of case management, an in-person assessment (using the Arizona Self-Sufficiency Matrix) is completed to determine baseline for goal-setting. Clients will then work with the case manager to set specific, measurable goals with timelines.

For unemployed clients, there will be 2-3 case management sessions per week. For those in school or working, there are 1-2 sessions per week.
 
Requirements for Strengths-Based Case Management include:
 
  • Must be 18 years or older
  • In need of a more stable living environment
  • Ability and willingness to set goals and work on these goals daily
  • Ability and willingness to identify personal strengths and challenges
  • Willingness and ability to maintain contact with case manager (no less than twice per month)
  • Employed, job searching, or in school pursuing certification/degree or a combination of two, equaling 40 hours weekly
